                      Since 1989, the Center for AIDS Research, Education and Services (CARES) has been transforming lives through care, research and community awareness.  As the Sacramento region’s leading HIV/AIDS care provider, we work tirelessly to educate at-risk populations, provide access to testing, and improve the lives of those infected with the disease.

                      With the equivalent of four full time doctors on staff, and a team of highly qualified nurse practitioners, our patients receive the care they need to lead active lives.  But, we don’t stop there.  CARES believes successful HIV/AIDS education and treatment should be a holistic approach, which is why we offer 20 different services related to HIV and STI infections:

                      Harm Reduction Services of Sacramento (HRS) works with injection drug users and other high-risk individuals in order to reduce the spread of HIV and other infectious diseases and promote increased quality of life and health among the IV-drug user community of Sacramento. HRS assists with community outreach, mobile clinic location choices, policy and practical advice, and education and counseling services.

                      The Sutter Hospital We CARE Program provides medical case management services targeting women and their families living with HIV/AIDS.  Services will include tracking of HIV+ pregnant women to ensure continued compliance with and access to primary medical care to reduce risk of HIV transmission to the unborn child. Sutter also provides medical case management services under the Minority AIDS Initiative (MAI) to outreach to African American and Hispanic HIV+ females who may be at risk of dropping out of care or have been identified as needing access to medical care.

                      The Effort has focused on overcoming threats to families by providing child and family therapy, crisis intervention, child abuse prevention, domestic violence prevention, and other health and social services. They have long recognized the effect the breakdown of the family has on the individual and the community. The Effort has developed quality mental health and social service programs that address the changing needs of our community.

                      UC Davis Children’s Hospital is the only medical facility in the Sacramento region that provides care for children who are exposed to or infected with HIV. The pediatric HIV Clinic provides comprehensive medical and support services, utilizing a team of specialized physicians, nurses, social workers, nutritionists and pharmacists. The goals of this team are to ensure that the children receive the highest quality medical treatment and achieve the highest level of well being possible.

                      The Ryan White Program at UC Davis provides pediatric primary medical care, pediatric medication adherence services, mental health treatment, medical transportation services, and medical case management services to children living with HIV/AIDS.

                      Client-focused and community-based, CommuniCare's HIV/AIDS medical case management services help people living with HIV/AIDS in Yolo County to maintain and enhance their independence and quality of life. The HIV/AIDS medical case management services assist clients in accessing appropriate resources through referrals and provides support for clients dealing with multiple sources of stress including physical illness, emotional well being, finances and social relationships. HIV/AIDS medical case management services are comprehensive and long term in scope.
